About This Location
Effective November 1, 2021, Washington Regional Crossroads Medical Clinic will become Baxter Regional Crossroads Medical Clinic. Baxter Regional is proud to welcome current providers Victor S. Chu, MD; Kevin T. Jackson, MD; Jason Froeschle, APRN; and Amanda Winford, APRN, as well as new provider Kim Cudworth, APRN.
To prevent any disruption in care during this transition, patients should complete and sign an Authorization to Release Medical Information Form.
