A social worker is available to assist patients with social or financial problems that may impact the patient’s care or recovery.

Baxter Health’s home health telemonitoring remotely monitors patients’ vital signs data and sends short surveys about current health statuses to the patients and caregivers. Clinicians can track patients’ conditions daily, which promotes fast follow-up and intervention.
